Getting started - partnership, co-production and defining the problem
One Northern Devon (OND) is a partnership of public and voluntary sector partners working together to improve wellbeing in North Devon & Torridge. It aims to reduce health and wellbeing inequalities through co-ordination of the activity of all partners involved to create a system change that is person-centred and place-focussed.
Designing your intervention
In January 2020, One Northern Devon launched ‘High Flow’, a personalised and holistic support programme for the 20 most intensive users of public services in Northern Devon. The High Flow programme aims to better meet the needs of these individuals and use the learning to improve future system response.
In July 2020 the Homelessness Multi-Agency Partnership was developed to deliver bespoke pathways to the most complex rough sleepers in Torridge. The partnership works under a Trauma Informed and Co-Produced framework to support rough sleepers towards sustainable accommodation and support plans based on individual needs, and not the needs of the wider organisations established to support them.
The transitional Partnership is supported by the One Northern Devon high flow strategic board.
Sustainability and system change
We use a monthly systems change index to highlight system change success or learning and escalate that to the senior strategic leaders within One Northern Devon. This is used as a challenge to those leaders to focus on their role within the ‘system’ and in turn create a MEAM system as opposed to a simple MEAM ‘intervention’.
